Attending your appointment

All participants will be required to complete a pre health questionnaire, all questionnaires are returned to HR or responsible manager after assessment. No confidential information is stored by RGF.

All assessments are carried out by registered nurses who are NMC registered.

Please bring:

  • Photo ID (e.g. passport or driving licence)
  • Spectacles/Lenses if worn for driving
  • Details of prescribed medications

Medical certificates will be provided after completion of medical assessment and returned directly to HR or responsible manager.

In the event that a medical certificate is not able to be issued, a referral will be made to HR/responsible manager outlining identified problems for further action by GP or occupational health department. Once this has been addressed and resolved to an acceptable standard a certificate may be issued by the nurse without the need for additional medical assessment.
